Home Health Care is providing skilled nursing, rehabilitation, and home health aide services on a part-time basis to home bound patients with a variety of medical conditions. These are delivered under the order of a physician.
Serve Link Home Care contracts with the Putnam County Health Department to provide the skilled nursing portion of care.
Other components of home health may include:
Medicare, Medicaid and the Veteran’s Administration often pay for these services. Serve Link is not currently accepting commercial insurance for home health patients.
Services may available for non-home bound individuals through the In-Home program if an individual qualifies through Medicaid. If not, private pay for homemakers services and personal care services is available through Serve Link.
