The Putnam County Health Department can provide vital records for only the state of Missouri. If you would like to obtain a copy of a birth or death certification, please complete an application and submit it in person at the health department, along with the required fee. The entire in-person process will likely take no more than ten minutes. Proof of ID is required for your visit!
If you are unable to visit us in person, you may also submit an application via mail – as long as it is notarized.
