Home Health and In-Home (RN) Programs

The Putnam County Health Department contracts with Serve Link Home Care to proved Home Health and
In-Home services to clients in Putnam County. Serve Link Home Care is an independent, not for profit
agency providing home health and in-home care to persons throughout north central Missouri. Most of their
services are delivered through a network of providers, like the health department, working under Serve Link’
s licensing umbrella.

Nurses employed by the health department work through this arrangement which enables us to provide
local nurses working in their own communities with the assistance of Serve Link staff to design and carry
out the care plan. Physical therapy for these patients is provided by the Putnam County Memorial Hospital.

Serve Link also does the billing for these patients and is Medicare licensed and accepts Medicaid through
Missouri Health Net. Many types of insurance are also accepted. Private pay and arrangements through
long term health insurance plans are also accepted by Serve Link.

Public Health Nursing

Occasionally, patients may not qualify for home health or in-home services and may still require nursing
assistance. The Putnam County Health Department is able to see an additional limited number of clients
depending on resources and staff availability. A doctor’s order is required for these nursing visits. We do not
have anyone available for homemaker/chore care needs but do maintain a “Stay at Home List” of local
residents who have requested to have their contact information made available to those who require such
assistance. A copy of the Stay at Home List can be obtained at the health department once a waiver has
been signed.
